Your Health Organization in Aiken, SC is seeking a Care Group Administrator to provide strategic leadership and oversee operations for our care groups. The role focuses on delivering high-quality, patient-centered care while ensuring regulatory compliance and staff development.

The position requires a Master’s in nursing (or equivalent experience) and a track record of executive leadership within healthcare.

Qualifications

• Master's degree in nursing strongly preferred; substantial relevant experience may be accepted in lieu of advanced degree.
• Minimum of 5 years of progressive leadership experience with at least 3 years in a senior management or executive role.
• Strong understanding of federal and state regulations governing hospice and palliative care.

Responsibilities

• Strategic Leadership: Develop current staff, ensure staff retention, and staff fulfillment; identify opportunities for program growth and expansion; stay abreast of industry trends; represent the organization externally as a thought leader.
• Operational Management: Oversee day-to-day operations for care groups; manage budget development and financial performance; ensure compliance with federal/state/local regulations; implement policies and procedures; optimize resources including staffing and technology.
• Quality & Compliance: Establish and maintain a robust quality improvement program; ensure adherence to clinical guidelines; lead accreditation and regulatory survey preparation; promote continuous learning.
• Recruit, hire, train, and retain a highly skilled and compassionate team of palliative and hospice professionals.
• Foster a positive work environment, provide leadership and professional development; conduct performance evaluations and feedback.
• Community & Stakeholder Engagement: Build relationships with partners, providers, referral sources; participate in outreach and education; collaborate with departments for seamless transitions.
• Patient & Family Experience: Champion patient-centered care; monitor satisfaction and implement improvements.

We are seeking a qualified Care Group Administrator in our Aiken office. This candidate should be compassionate with excellent management skills to assist and manage our Care Groups

This role would be in person in our local offices daily. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

Position Summary:

The Care Group Administrator is a dynamic and experienced leader responsible for the strategic vision, operational excellence, and overall success of our care group preventing and reducing unnecessary ER visits and hospitalizations .This executive will ensure the delivery of high-quality, patient-centered care in accordance with all regulatory standards and organizational policies. The Care Manager will foster a culture of compassion, collaboration, and continuous improvement, leading a dedicated team of healthcare professionals to achieve outstanding outcomes for our patients and their families.

Key Responsibilities:

• Strategic Leadership:
  • Develop current staff, ensuring staff retention, and staff fulfillment
  • Identify and pursue opportunities for program growth, innovation, and expansion of services to meet community needs.
  • Stay abreast of industry trends, best practices, and regulatory changes
  • Represent the organization externally as a thought leader
• Operational Management:
  • Oversee all aspects of day-to-day operations for care groups ensuring efficiency, effectiveness, and compliance.
  • Manage fiscal operations, including budget development, monitoring, and financial performance.
  • Ensure compliance with all federal, state, and local regulations, including Medicare, Medicaid, and other relevant accreditation bodies (e.g., Joint Commission).
  • Develop and implement policies and procedures that promote patient safety, quality of care, and operational efficiency.
  • Optimize resource allocation, including staffing, equipment, and technology.
• Quality & Compliance:
  • Establish and maintain a robust quality improvement program, regularly monitoring key performance indicators and implementing corrective actions as needed.
  • Ensure adherence to all clinical practice guidelines and ethical standards.
  • Lead accreditation and regulatory survey preparation and response.
  • Promote a culture of continuous learning and evidence-based practice
• Recruit, hire, train, and retain a highly skilled and compassionate team of palliative and hospice professionals.
• Provide strong leadership, mentorship, and professional development opportunities for staff.
• Foster a positive and supportive work environment that promotes teamwork, collaboration, and open communication.
• Conduct performance evaluations and provide constructive feedback to staff.

  • Build and maintain strong relationships with community partners, healthcare providers, referral sources, and other stakeholders.
  • Participate in community outreach and education initiatives to raise awareness of palliative and hospice services.
  • Collaborate with other departments within the organization to ensure seamless patient transitions and coordinated care.
• Patient & Family Experience:
  • Champion a patient-centered approach to care, ensuring that the needs and preferences of patients and their families are at the forefront of all decision-making.
  • Monitor patient and family satisfaction and implement strategies for continuous improvement.
