Qualifications

• Medical Assistant education, training, or relevant clinical experience.
• Ability to obtain vital signs, document patient information, and work accurately in an EHR.
• Strong communication, organization, and follow-through skills.
• Ability to communicate with patients, providers, pharmacies, and care-team members.
• Ability to balance in-office clinical duties with patient outreach and care coordination.
• Willingness to complete EPIC Moodle care-management training and required assessments.
• Successful completion of background checks and pre-employment requirements.

Responsibilities

• Prepare examination rooms, document vital signs, medical histories, allergies, and current medications.
• Assist providers with examinations and procedures within training and scope.
• Perform specimen collection, phlebotomy, injections, and vaccinations as trained.
• Coordinate follow-up appointments, diagnostics, and referrals.
• Identify barriers to care and coordinate appropriate resources.
• Support transitions of care after hospitalizations or ED visits.
