Position Overview As a Care Coordinator, you will play a vital role in supporting our members by identifying their evolving needs and creating personalized care plans. You'll work closely with members, families, healthcare providers, and community resources to ensure seamless care and optimal well-being.
What You’ll Do:
Manage a caseload of members, conducting assessments and developing individualized care plans.
Coordinate services, provide oversight to care plans.
Serve as a trusted advocate, educating members and their families about available resources and benefits.
Work collaboratively with physicians, service providers, and other key stakeholders.
Participate in wellness initiatives, community outreach, and sales support activities.
Maintain accurate documentation of all member interactions and care plans.
Our Ideal Candidate Will:
Have a bachelor’s degree in healthcare, nursing, social work, COTA, PTA, or a related field.
Have at least three (3) years of experience working with older adults in a healthcare, AL, SNF or case management setting.
Strong assessment, planning, and organizational skills.
Knowledge of Medicare and Long-Term Care Insurance.
· Positive, enthusiastic attitude with demonstrated interest in providing exceptional service and building meaningful relationships.
· A compassionate and solution-oriented mindset with excellent communication skills.
A valid driver’s license and the willingness and ability to travel regionally to support members.
