Job summary

Clinical & Support Options, Inc. (CSO) in Western Massachusetts is seeking a Care Coordinator for the Intensive Care Coordination (ICC)/Family Based Intensive Treatment (FIT) team in Pittsfield.

This role focuses on wraparound care planning that respects family culture and strengths, coordinating services to support youth aged 3-21. The position offers a starting salary of $38,000–$44,000 per year, with opportunities for professional development and a collaborative, flexible work environment

Qualifications

• Master's or Bachelor's degree in a mental health field.
• Associate's degree or high school diploma with at least 5 years of relevant experience with 3–21 year-olds.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate services for children and youth under 21 to develop an integrated Care Plan.
• Facilitate Care Planning and wraparound model learning.
• Liaise with partner agencies, state agencies, and school personnel.
• Meet with youth and families at home, in the office, or in the community.
• Provide referrals and linkages to services.
• Support caregivers in effective advocacy.
• Identify and engage the family’s natural supports on the team.
• Be flexible for occasional evening/weekend shifts.
