Job summary

Morrison Center is seeking Adult Care Coordinators to support adults with behavioral health, intellectual and developmental disabilities in Westbrook and Oakland, Maine. The role emphasizes compassionate care, collaboration with an interdisciplinary team, and delivering person-centered coordination.

The position involves home and community visits, case management, and close coordination with providers, with a potential for some hybrid work.

Qualifications

  • Bachelor's degree in a related field is required.
  • MHRT/C eligibility is required or in process.
  • Minimum of 1 year of paid experience with behavioral health or developmental disabilities.
  • Strong organizational, writing and time management skills.
  • Experience in care coordination preferred.

Responsibilities

  • Maintain an active caseload with monthly member contact.
  • Conduct home, community, and office-based visits to support engagement.
  • Lead interdisciplinary team meetings and coordinate communication with providers and partners.
  • Provide comprehensive care coordination for Behavioral Health Home participants.
  • Connect members with healthcare, housing, employment, benefits, transportation, and resources.
  • Monitor progress and adjust care plans as needed.
  • Complete annual and 90‑day assessments per DHHS and program requirements.
  • Promote self-advocacy and independence; identify service gaps and address barriers.
  • Document accurately and timely per DHHS, MaineCare, and Morrison Center policies.
  • Participate in quality improvement, audits and compliance initiatives.
  • Serve as liaison among members, families, providers, and community agencies.
  • Travel within the community to support members and coordinate services.

Morrison Center is seeking Adult Care Coordinators for our Westbrook and Oakland, Maine locations. Candidates should be dedicated, compassionate professionals who are passionate about empowering adults with behavioral health, intellectual and developmental disabilities.

*$1,000 sign-on bonus to be paid with first paycheck.*

Requirements Of The Adult Care Coordinator
  • Bachelor’s degree in a related field
  • MHRT/C eligible
  • A minimum 1 year of paid experience working with individuals with behavioral health, intellectual and/or developmental disabilities (e.g., DSP, BHP, RBT, etc.)
  • Strong organizational, writing, and time management skills
  • Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities
  • Previous experience in care coordination is preferred but not required
  • Microsoft Office Suite

Responsibilities Of The Adult Care Coordinator
  • Maintain an active caseload and monthly contact with members.

  • Lead interdisciplinary team meetings and coordinate communication among providers, supports, and community partners.
  • Provide comprehensive care coordination services to adults enrolled in the Behavioral Health Home program.
  • Connect members with healthcare, housing, employment, benefits, transportation, and community resources.
  • Monitor member progress and adjust care plans as needed.
  • Complete comprehensive, person-centered, annual, and 90-day assessments in compliance with DHHS and Behavioral Health Home requirements.
  • Promote informed decision-making, self-advocacy, and independence.
  • Identify service gaps and collaborate with partners to address barriers to care.
  • Maintain accurate, timely documentation in compliance with DHHS, MaineCare, Behavioral Health Home, and Morrison Center requirements.
  • Participate in quality improvement, audit, and compliance initiatives.
  • Serve as a liaison among members, families, providers, and community agencies.
  • Travel within the community to support members, attend appointments, and coordinate services.
