Position Summary

The Health Connections Care Management Program supports families by helping eligible children access and coordinate medical, behavioral health, and community-based services to promote optimal health and well-being. The Care Coordinator Supervisor provides clinical and operational supervision to a team of up to seven Care Coordinators, ensuring effective care management practices, accurate documentation, compliance with billing requirements, and high-quality service delivery.

Reporting to the Director of Healthcare Management, the Care Coordinator Supervisor oversees Care Coordinator activities, supports staff development, and promotes collaboration across Children's Aid programs and external community partners. This role represents Health Connections within the community and helps ensure services are delivered in alignment with Children's Aid, Health Home, regulatory, and quality assurance standards.

Essential Duties and Responsibilities
Team Leadership & Supervision
  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Provide regular individual and group supervision to a team of up to seven (7) Care Coordinators.
  • Oversee Care Coordinator cases and care management activities to support effective service delivery.
  • Conduct regular and timely performance evaluations for Care Coordinators.
  • Provide training and support to new staff and ongoing professional development for existing staff.
  • Provide coverage for Care Coordinator responsibilities as needed.
Care Management Oversight & Quality Assurance
  • Manage, review, and monitor client records to ensure compliance with internal and external quality assurance standards, Children's Aid requirements, and Health Home policies.
  • Ensure required documentation is completed accurately and within established timelines.
  • Review and approve CANS-NY assessments completed by assigned Care Coordinators and complete assessments when needed.
  • Monitor care management activities to ensure established program requirements and service goals are met.
  • Work closely with the Quality Assurance Manager to support quality practices, service delivery standards, and audit preparation.
  • Compile reports and provide requested information to support program operations and oversight.
Compliance, Billing & Operations
  • Ensure that billing requirements are met and appropriately documented to support reimbursement.
  • Maintain confidentiality, comply with HIPAA, and other applicable privacy requirements.
  • Support program goals and established enrollment targets as part of the Health Connections team.
  • Participate in program, division, and agency-wide meetings, events, trainings, and activities.
  • Provide on-call coverage as needed.
Collaboration & Community Engagement
  • Participate in interdisciplinary team meetings to support care plan development.
  • Serve as a liaison between Children's Aid programs and Health Connections to support coordinated services.
  • Represent Health Connections at community events, partner organizations, and external meetings.
  • Develop and maintain relationships with community resources to support family needs and identify potential referral opportunities.
  • Conduct fieldwork as needed, including possible evenings or weekends.
  • Perform other duties as assigned.
Minimum Qualifications
  • Master's degree in social work, Counseling, or related field required.
  • LMSW, LCSW, LMHC, or related professional credential preferred.
  • Two (2) years of supervisory experience required.
  • Experience providing care management, case management, or related services preferred.
  • Ability to communicate effectively, organize priorities, manage multiple responsibilities, and support a fast-paced service environment.
  • Ability to provide on-call coverage, support field-based work, and travel throughout the community as needed.
  • Proficiency with Microsoft Office applications, including Outlook, Word, and Excel; experience with GSI or similar care management systems preferred.
  • Bilingual Spanish/English preferred.

Experience
