RiseWell Community Services is seeking a full-time Care Coordinator to join the Suffolk State Aid program. In this role, you will be responsible for providing care coordination and health promotion to individuals with severe mental illness.
LOCATION: Remote/Fieldwork - staff are expected to travel in the field to visit clients
DEPARTMENT: Care Coordination
HOURS: 35 Hours per week
PAY: $33,000 Annually
QUALIFICATIONS:
Minimum of a High School Diploma. Experience serving people with mental illness, developmental disabilities, or alcoholism or substance abuse preferred. Clean, valid NYS Driver’s License required. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
JOB FUNCTION:
Provide care coordination and health promotion to individuals with severe mental illness. Oversee and coordinate access to services including behavioral, medical health, and social service supports. Assist individuals in maintaining wellbeing. Complete all required documentation according to standards, and within 48 hours of service delivery.
