Job summary

Northside Hospital in Atlanta is seeking a Care Coordinator to determine appropriate care settings for patients across age groups and to support effective discharge planning in a fast-paced acute care environment.

Responsibilities include psychosocial assessment, crisis intervention, and coordination with families, staff, and community agencies to ensure appropriate utilization of hospital resources. Schedule includes 8-hour and 10-hour shifts; regular hours 8 am–4:30 pm.

Qualifications

• REQUIRED: MSW or Bachelor’s degree in health-related field.
• PREFERRED: Three years of experience in case management, social services or related field.
• One year of experience in utilization review.
• Certified Case Manager.

• Coordinate care for neonate to geriatric patients to determine appropriate setting and hospitalization.
• Involves discharge planning and utilization review in a progressive acute care setting.

Responsibilities

The Care Coordinator will be responsible for working with neonate, adolescent, adult, and geriatric patients, families, medical staff and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post hospital care needs. Provides the necessary interventions in order to assure effective and efficient clinical resource management. Provides intervention regarding adjustment to hospitalization, crisis intervention and psychosocial assessment. Will be actively involved in Discharge Planning and Utilization Review in a progressive acute care setting.

Schedule:

• Monday, Thursday, Friday (8 hour (OR) 10 hour shifts)

Qualifications

REQUIRED

• Masters of Social Work (MSW) OR Bachelor’s degree in health-related field.

PREFERRED

• Three (3) years of experience in case management, social services or related field.
• One (1) year of experience in utilization review.
• Certified Case Manager.

Work Hours:

8 am -430 pm
