REACH, which stands for REadmission Advocates Collaborating in Healthcare, is an Orlando Health program providing care management extension post-discharge. It connects patients and families with community resources, referrals, and services to facilitate healthcare management and improve quality of life. REACH services are available at all Orlando Health hospitals.

Department: Corporate Care Management/REACH Team

Shift/Status: PRN/Pool - 2 to 3 days per week

The Social Worker I collaborates with the clinical team to identify patients who can benefit from care coordination, assessing risk factors, and providing services such as care management and preventative care.

Responsibilities

Essential Functions

• Ensure continuity of care across inpatient, emergency, and outpatient settings, including discharge planning and follow-up care.
• Build effective relationships with the Care Management Team to engage patients and families, advocating and supporting their needs.
• Monitor patient progress daily and adjust discharge plans as needed, prioritizing high-risk patients.
• Educate patients and families about chronic illness management, healthcare system navigation, and self-management strategies.
• Identify support needs, develop action plans, and guide patients and families through self-management and resource utilization.
• Use IT resources to facilitate reporting, tracking, and improving health outcomes.
• Participate in clinical outcome measurement and population health strategies.
• Maintain compliance with policies, standards, and attendance requirements.
• Provide age-appropriate care, understanding growth, development, and legal considerations.

Qualifications

Education/Training

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field.

Licensure/Certification

• BLS certification.

Experience

• At least 1 year of clinical experience or healthcare internship.
