Qualifications

  • Bachelor’s in counseling, psychology, social work, or related field required.
  • Valid Tennessee Driver’s License and acceptable MVR needed.
  • Experience in behavioral health preferred.

Job description

Job Title : Care Coordinator, Safety Net (Antioch, TN)

Schedule : Monday-Friday, 8 : 00am-4 : 30pm (Onsite)

Salary : $40,900 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available.

JOB SUMMARY : Provide care coordination to consumers in Safety Net services.

RESPONSIBILITIES :

  • Continual assessment of consumer’s mental health treatment needs.
  • Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
  • Ensure consumer’s psychiatric needs are planned for and coordinated.
  • Facilitate education of consumer and / or involved collaterals to psychiatric diagnosis and appropriate treatment options.
  • Respond to urgent / crisis consumer needs as appropriate to position with assistance of clinic and crisis services.
  • Involve family members and friends to help consumers obtain Safety Net services.
  • Coordinate efforts with all staff and Safety Net nurse and providers.
  • Assist the consumer with applying for entitlements as needed.
  • Assist with Safety Net documentation.
  • Assist consumers with transportation for safety net appointments including providing transportation for them to and from appointments.
  • Keep consumer record updated with most current address and coverage information.
  • Assist Consumers as needed with application and renewal requirements for Safety Net coverage.
  • Consistently treat consumers and co-workers with respect.
  • Develop and maintain community relations through prompt follow through with contacts.
  • Meet productivity standards set forth in the Agency’s policies and procedures.

REQUIREMENTS :

  • Bachelor’s Degree in a health-related field of counseling, psychology, social work, or other behavioral health field required.
  • Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.

  • Advocates for consumer needs to improve or maximize their daily functioning.
  • Demonstrates effective communication skills with supervisor, team members and other agency employees.
