Job description

The Community Well Care Coordinator must oversee care coordination and complex case management functions for PathWays to Aging members living in the community who are not receiving Home and Community Based Services (HCBS) or designated as Nursing Facility Level of Care (NFLOC). The candidate must be a registered nurse or a similar medical professional with extensive experience in providing care coordination to members aged 60 and above. This position requires local travel and reports directly to the Health Services Director to maintain the care coordination program. Responsibilities include overseeing care coordination teams, developing and implementing care plans, and managing activities related to physical and behavioral health care services. The Care Coordinator will collaborate with various stakeholders including the Health Services Director, Service Coordinator Administrator, Medical Director, Provider and Member Services Managers, and State staff, providing input at State-level meetings as needed.

Job Title: Community Well Care Coordinator

Location: Remote (Indiana)
Must be an Indiana Resident

Hours: Monday - Friday, 8am-5pm

Pay: $38-48 hourly | Weekly pay

Primary Responsibilities:

  • Develop and support staff within the department or region, including mentoring and goal setting
  • Establish clear performance goals, communicate expectations, and ensure accountability
  • Understand and meet the contractual requirements to address members' unique needs
  • Travel locally as required
  • Identify, structure, and prioritize process improvement projects, implementing necessary changes
  • Ensure standardized workflow processes, including performance audits, quality reviews, and compliance
  • Assess, plan, and implement individualized care strategies aimed at the least restrictive level of care
  • Identify and refer members to social service programs, including financial, psychosocial, community, and state services
  • Communicate health-related information to stakeholders to ensure coordinated, timely care
  • Advocate for patients and families to support their needs and choices
  • Participate in training and coaching of direct reports
  • Conduct bi-annual field visits with team members to observe, teach, address issues, and foster relationships
  • Collaborate across departments and with various stakeholders including Medical Directors, Site Directors, and others

Required Qualifications:

  • Resident of Indiana
  • BSN or equivalent experience
  • Registered Nurse license in Indiana
  • Experience in community health settings
  • Valid Driver's License
  • Knowledge of Indiana Medicaid, Medicare, and long-term care
  • Experience coaching or mentoring staff
  • Proficiency with Microsoft Word and Windows environment

Preferred Qualifications:

  • 3+ years of healthcare leadership in case management
  • Background in managed care
  • Certified Case Manager (CCM)
  • Experience with long-term social supports and utilization review
