Rochester Regional Health is seeking a Care Coordinator RN at Greenfield Medical Campus, Canton, NY. The role supports chronically ill or high-risk patients, coordinating with clinicians and resources to ensure patient-centered care and appropriate utilization of services.

The position requires an AAS in Nursing and NYS RN license, with preferred BS in Nursing and 3–5 years in clinical or community settings. On-site schedule is Mon-Fri, 8-4:30, about 37.5 hours weekly.

Qualifications

• AAS in Nursing is required.
• Valid NYS RN license required.
• BS in Nursing and care coordination experience preferred.

Responsibilities

• Care Coordination – Identify patients’ needs, create and adjust individualized care plans, educate patients, address barriers, and coordinate with community and regional health services.
• Patient Education & Support – Provide tailored education on self-management, medications, and tests; support high-risk patients and lead patient groups or shared medical appointments.
• Care Delivery – Ensure safe nursing care, triage calls, facilitate team huddles, collaborate with community resources to extend care.
• Quality & Compliance – Collect and report patient data, participate in team conferences, and engage in quality improvement initiatives.
