Our team travels directly to patients who reside in Assisted Living, Memory Care, and Group Home communities throughout Minnesota, Wisconsin, and Florida, supported by remote clinical operations and administrative colleagues at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.
Join our team as a Care Coordinator where you will manage Department of Human Services and CMS required activities for Minnesota Senior Health Options members living in the community and assisted living facilities. The role is primarily remote with regular travel to serve members and the community.
Full‑time, day shift hours: 8:00 am to 5:00 pm Monday–Thursday and 8:00 am to 3:00 pm Friday. No evenings, weekends, or holidays.
Approximately 50–70% travel within the Little Canada and Mahtomedi areas and 30–50% remote work.
$65,000 – $75,000. Salary commensurate with experience.
