Our team travels directly to patients who reside in Assisted Living, Memory Care, and Group Home communities throughout Minnesota, Wisconsin, and Florida, supported by remote clinical operations and administrative colleagues at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview

Join our team as a Care Coordinator where you will manage Department of Human Services and CMS required activities for Minnesota Senior Health Options members living in the community and assisted living facilities. The role is primarily remote with regular travel to serve members and the community.

Schedule

Full‑time, day shift hours: 8:00 am to 5:00 pm Monday–Thursday and 8:00 am to 3:00 pm Friday. No evenings, weekends, or holidays.

Location

Approximately 50–70% travel within the Little Canada and Mahtomedi areas and 30–50% remote work.

Salary Range

$65,000 – $75,000. Salary commensurate with experience.

Responsibilities

• Coordinate face‑to‑face visits
• Manage the Elderly Waiver
• Conduct annual assessments, including Personal Care Assistance assessments for community members and customized tools for assisted living residents
• Review health needs, set goals, and develop individualized care plans
• Connect members with community and state resources and services
• Complete required documentation
• Collaborate with medical care teams to meet quality measures and utilize utilization management tools to achieve value‑based goals
• Support members during transitions of care and collaborate with their care team to ensure safe discharge and follow‑up plans

Qualifications

Education, Certification, and Experience

• Minnesota Registered Nurse license
• Or current Minnesota Licensed Social Worker (new grads encouraged)
• One or more years of experience working with the geriatric or certified disabled population in case management, care coordination, home care, group homes, nursing homes, TCU, or assisted living settings preferred
• Valid driver’s license
