Description

The Care Coordinator works under the direction of the Director of Nursing and is responsible for the timely completion of accurate resident assessments and interdisciplinary care plans that meet state and federal guidelines. The Care Coordinator is responsible for identifying resident acuity, determining specific care needs, and communicating those needs and expectations to families and responsible parties. The Care Coordinator oversees the clinical reimbursement of the facility and ensures Medicare and Medicaid compliance. Additionally, they provide oversight of the Restorative Nursing Program.

Requirements
  • Active, Non-Probationary RN or LPN License
  • Active, Class-Instructed CPR Certification
  • Background Screening, Licensure Verification (if applicable), Reference Check, Drug Testing
  • Tuberculosis Testing
  • Employee Health Screening (Conducted after Hire)
  • Ability to Move, Lift, and Transfer a minimum of 50 Pounds
  • Must be able to Speak, Read, Write, and Understand English
