Job summary

CHI Health Schuyler seeks a Care Coordinator RN to advocate for patients and families, oversee utilization review, and manage care progression in the Clinical Resource Management department. Qualifications

  • Graduate of an accredited school of nursing; RN licensed in NE.
  • Minimum 2 years acute hospital clinical experience (or Master's + 1 year).
  • Registered Nurse: NE (RN: NE) licensure required.
  • Basic Life Support (BLS) CPR within 90 days.

Responsibilities

  • Utilization Review & Care Progression: Apply evidence-based guidelines to manage inpatient resources and ensure appropriate care level.
  • Multidisciplinary Discharge Planning: Develop and execute discharge plans with care teams and post-acute providers.
  • Readmission Risk Mitigation: Identify high-risk patients and implement 30-day strategies.
  • Patient Advocacy & Support: Address cultural, language, or socioeconomic barriers and educate patients/families.
  • Regulatory Compliance & Documentation: Ensure care transitions are documented per policies and regulatory standards.

Every day you will perform comprehensive discharge planning assessments and facilitate multidisciplinary care plans that align with evidence-based clinical guidelines. You will actively engage with the healthcare team to manage inpatient resource utilization, identify readmission risks, and implement effective 30-day mitigation strategies. Your daily workflow will involve maintaining clear communication with stakeholders, delivering patient notifications, providing education, and documenting care transitions to ensure seamless continuity of services from the hospital to post-acute settings.

You should be a collaborative team player who can effectively prioritize tasks and leverage your expertise to bridge the gap between patient needs and efficient, regulatory-compliant care.

  • Utilization Review & Care Progression: Perform rigorous utilization review activities and apply evidence-based clinical guidelines to manage inpatient resources and ensure patients receive the appropriate level of care.
  • Multidisciplinary Discharge Planning: Facilitate and oversee the development and execution of comprehensive discharge plans by collaborating with the healthcare team, post-acute providers, and patients to ensure a safe, efficient transition.
  • Readmission Risk Mitigation: Proactively identify patients at high risk for readmission and implement targeted, 30-day intervention strategies to improve long-term health outcomes.
  • Patient Advocacy & Support: Serve as a dedicated advocate for patients and their families by identifying and addressing cultural, socioeconomic, or spiritual barriers to care and delivering essential patient education.
  • Regulatory Compliance & Documentation: Ensure all care coordination activities, patient notifications, and transitions are documented accurately in accordance with hospital policies and Federal/State regulatory standards.
Job Requirements

Required -

  • Graduate of an accredited school of nursing upon hire and Minimum two (2) years of acute hospital clinical experience or
  • Masters of Case Management or Nursing field upon hire and 1 year of experience
  • Registered Nurse: NE (RN:NE) upon hire and
  • Basic Life Support - CPR (BLS-CPR) within 90 Days
Preferred
  • Bachelors Of Science Nursing upon hire and At least five (5) years of nursing experience.
  • Certified Case Manager (CCM) N/A upon hire
  • Accredited Case Manager (ACM) N/A upon hire
