Position Details
This position is on site in Seattle, WA.

How You’ll Serve Our Patients

The Care Coordinator plays a critical role in patient admissions, transitions, and provides administrative and clinical support to the treatment teams. Responsibilities include ensuring patients complete treatment plan assessments and engagement surveys. You will serve patients daily in various ways:

  • Support the care team by identifying referrals for varying levels of care/treatment and coordinating these services.
  • Coordinate discharge and transfers to other treatment facilities.
  • Submit daily attendance data from sign-in/out logs or midnight census.
  • Update treatment teams and patient tracker, set expired reasons, and ensure data consistency across systems.
  • Assist with virtual doctor appointments by pulling patients and setting up equipment.
  • Manage admission schedules and send welcome emails or information to patients/families.
  • Schedule rounds and coordinate transportation.
  • Manage Preventive Quality Indicators (PQIs) for timeliness and accuracy.
  • Complete research assessments (TPA) and Patient Engagement Surveys (PES) with patients and families.
  • Coordinate with outpatient providers, send welcome letters at admission, and update contact information.
  • Update electronic medical records with patient movements, transfers, discharges, and involuntary statuses.
  • Facilitate FMLA/disability paperwork and obtain work and school absence letters.
Must Haves
  • At least 2 years of experience in a clinical administrative setting (required).
  • Bachelor’s degree in psychology, social work, or related field (preferred).
