Job summary

Crossroads Turning Points in Pueblo West, CO is seeking a behavioral health professional to develop care plans, assess patient needs, and coordinate services across hospitals and agencies in a residential setting.

Role requires 2+ years in substance misuse/mental health, CAT and case management certifications, and strong computer skills; driving is required. Salary is $20.25/hour with benefits and career advancement opportunities.

Qualifications

• An associate or bachelor’s degree in a behavioral health-related field is preferred.
• Obtain the Certificate of Addiction Technician (CAT) certification within 18 months of your hire date.
• Obtain case management certification within 90 days of hire.
• Minimum of two years of experience in substance misuse and mental health.
• Advanced computer skills are required.
• CPR and First-Aid certification (agency pays for training and renewals).
• Must be able to take vital signs and refer to medical staff when necessary.
• Valid driver’s license and good driving record.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Responsible for the development of care planning and patient needs assessments.
• Facilitates life skills groups by the CTP policies and procedures.
• Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs.
• Document admissions, care management needs, discharge, and discharge planning.
• Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake.
• Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators.
• Assists treatment team with patient intake, transfer, and discharge assessments.
• Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients' needs.
• From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, religious, etc.).

• Must have a good understanding of substance use, mental health, and treatment delivery systems.
• Advanced computer skills are required.
• CPR and First-Aid certification (the agency pays for initial training and renewals).
• Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary.
• Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire).
• Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance use related.
