Hourly Pay Range: $30.46 - $45.69. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
Position Highlights
- Position: Care Coordinator MSW - Care Coordination
- Location: Arlington Heights, IL
- Full Time, 40 hours/week
- Hours: 8am - 4:30pm, Rotating weekends
- Required Travel: No
What You Will Do
- Implement assessments and care based on the developmental stages of the patient, involving family/guardians in the care plan, and conducting education tailored to the patient's developmental and high-risk needs.
- Provide direct social work services and grief counseling to patients and families to improve coping with illness-related issues.
- Identify strengths and resources for patients and families; document actions, interventions, or referrals.
- Collaborate with multidisciplinary teams for discharge planning, ensuring transitional care plans meet patient needs and payor requirements.
- Coordinate care for high-risk populations, serve as a resource post-discharge, and facilitate community linkage and referrals to reduce readmissions.
What You Will Need
- Master's Degree in Social Work from an accredited institution.
- Licensure to practice social work in Illinois: LSW/LCSW or eligible; must obtain LSW within 90 days of hire.
- At least two years of recent clinical experience in social work, preferably in acute care, mental health, or medical social work.
- Current CPR certification is a plus.
