Care Coordinator Manager (Behavioral Health)

Position Summary

The Manager of Care Coordination oversees services provided by the Care Coordination program, including supervising Supervisors and Coordinators. This role ensures compliance with HFS rules and collaborates with the Manager of Crisis Care to coordinate care for mutual clients. The manager also engages in community provider education, program development, and organizational policy. As part of the Leadership Team, they contribute to budget development and administrative initiatives.

Responsibilities
  • Hire, train, and evaluate Supervisors and Coordinators.
  • Implement best practices and Evidence-Based Practices for Systems of Care and Wraparound.
  • Collaborate with community providers and ensure program metrics and state reporting requirements are met.
  • Monitor caseloads and ensure Medicaid standards and contractual requirements are fulfilled.
  • Complete all required training and adhere to agency policies.
Skills and Qualifications
  • Strong organizational and communication skills.
  • Ability to develop community relationships and conduct training.
  • Understanding of Systems of Care and Wraparound principles preferred.
Education and Experience
  • Master's Degree in Social Work, Counseling, Psychology, or related field.
  • Valid IL license (LCSW or LCPC).
  • Two years of supervisory experience.
Requirements
  • Must be at least 21 years old.
  • Valid driver's license and proof of automobile insurance.
Additional Information

All employees must align with CARE principles and complete TCI training. Ongoing refresher training is required biannually and annually for physical and written assessments, respectively.
