Job Description

We are seeking Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team! This is a remote Care Coordinator position working with Medicare patients with multiple chronic conditions. Hours are from 8:30am CST to 5pm MST.

Job Details

• Start Date: 3 Full-Time Positions available for June and July

Qualifications:

• Minimum 1+ year experience working with Medicare patients with multiple chronic conditions as an LPN/LVN
• Ability to multitask, navigate EHR platforms, internet, and Microsoft Office (Excel, Word)
• Excellent communication skills via email and phone; ability to document time accurately and ensure patients receive required care services
• Positive attitude, self-starter, receptive to supervision and instruction

Responsibilities:

• Assist physicians in managing Medicare patients with chronic conditions
• Spend at least 20 minutes per month per patient on care management activities, including wellness checks, education, coordination, and follow-up
• Manage a caseload of 12-14 patients daily, working telephonically from home
• Engage patients to review and update care plans, promote healthy lifestyles, and close care gaps
• Review medication adherence and interactions, and coordinate with healthcare providers
• Document activities in the EHR and monitor trends for quality improvement
