Job Description

We are seeking Licensed Practical Nurses (LPN) or Licensed Vocational Nurses (LVN) to join our team! This remote Care Coordinator position involves working with Medicare patients who have multiple chronic conditions. Hours are from 8:30 am CST to 5 pm MST.

Job Start Dates: 3 full-time positions available for June and July start dates.

Qualifications
  • At least 1 year of experience working with Medicare patients with multiple chronic conditions as an LPN/LVN.
  • Ability to multitask, navigate EHR platforms, use the internet, and Microsoft Office (Excel, Word).
  • Excellent communication skills via email and phone, with the ability to document time spent with each patient and ensure service delivery meets billing requirements.
  • A positive attitude, self-starter mindset, and willingness to follow guidance from supervisors.
Responsibilities
  • Assist physicians treating Medicare patients with chronic conditions.
  • Spend a minimum of 20 minutes per month on each patient's case, including wellness checks, education, care coordination, appointment assistance, and referrals.
  • Manage a caseload of 12 to 14 patients daily, working telephonically from home.
  • Engage patients to review and update their care plans, promote healthy lifestyles, and close gaps in care.
  • Review medication profiles, address adherence and interactions, and oversee medication self-management.
  • Document all activities accurately in the EHR and submit daily reports.
  • Coordinate with healthcare providers, community resources, and other health system departments as needed.
  • Potential future duties include conducting health risk assessments and reassessments.
