Job summary

Charlotte Hungerford Hospital in Torrington, CT, seeks a Medical Social Worker to join the Care Management team. The role focuses on coordinating care, supporting patients and families, and using your MSW expertise to improve health outcomes within a community hospital setting.

Ideal candidates will have at least 3 years in medical social work, CT LMSW or eligibility, and familiarity with Epic and related health IT tools.

Qualifications

• Masters in Social Work from an accredited school of Social Work.
• Minimum of 3 years of Medical Social Work experience. Acute Care experience preferred.
• Maintains current license as Master of Social Work in the State of CT (LMSW). Case Management certification ACM or CCM Strongly preferred.

Qualifications
