Behavioral Health Care Partners is seeking a Care Coordinator in Newark, Ohio to provide care coordination for adults with mental health and substance use issues. The role includes monitoring client needs and ensuring delivery of required treatment.
Responsibilities include coordinating with other service providers and maintaining documentation. A high school diploma is required, with 1–3 years in care coordination; an associate degree is preferred for advancement.

Our offices are located at 65 Messimer Drive in Newark, Ohio or 8402 Blackjack Road in Mount Vernon. Both are a short 30-minute scenic commute from Columbus, Zanesville, and Lancaster.
Candidates must possess strong written and oral communication skills and the ability to collaborate with other service providers. High School Diploma with one to three years of care coordination experience for individuals with mental health or substance abuse issues. Preferred Associates Degree in Human Services or related field. Qualified Mental Health Specialist (QMHS). Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) preferred. State of Ohio Driver’s License. BLS/CPR certification required. Basic computer, phone and typing skills are necessary for all positions.
