Buckeye Health Plan is seeking a remote Case Manager supporting Long Term Care (LTC) care coordination. You will conduct member outreach by phone or home visits, discuss care plans, and coordinate with providers to ensure service delivery and follow‑ups.
Applicants should reside in Ohio, with occasional travel to Ohio offices (up to 10%). Schedule is 8am–5pm ET, may include overtime, evenings and weekends. Requires a High School diploma and 2–4 years of related experience.

Pay Range: $20.39 - $34.71 per hour
