Centene is seeking a Care Coordination Associate to support the Foster Care/DCYF team in Bedford, NH or remotely from NH locations. You will assist care managers with outreach, update monitoring care plans, and coordinate with healthcare providers to ensure continuity of care and member satisfaction.
The role requires a high school diploma or GED and 2–4 years of related experience. Centene offers competitive pay and a flexible work approach with remote, hybrid, field or office arrangements.

Works with senior care management team to support care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Serves as a liaison alongside care managers and providers to ensure proper coordination of care for members and interacts with members by performing member outreach telephonically or through home-visits.
This role will provide administrative support for the Foster Care/DCYF team, including assisting Care Managers and Care Navigators with tasks such as emailing and updating monitoring care plans. The person will also maintain assigned monitoring care plans and complete member outreach to support comprehensive assessments as needed.
Requires a High School diploma or GED.
Requires 2 - 4 years of related experience
Pay Range: $20.39 - $34.71 per hour
