A leading healthcare provider seeks an entry-level Care Coordinator to support member care management. The role involves conducting outreach to members, coordinating care activities, and ensuring documentation compliance. This fully remote position offers a competitive starting pay of $25.00 per hour, with potential benefits including medical, dental, and 401(k). The anticipated application deadline is June 30, 2025.
*Description*
Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, and continuity of care/member satisfaction is achieved.
Interacts with members by performing outreach via telephone or home visits, and documents care/service plans and activities.
* Provides outreach to members via phone or home visits to engage members, discuss care plans, resources, questions, concerns, and provide ongoing education throughout care/service.
* Coordinates care activities based on the care plan/service plan, working with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress.
* Serves as support for member and/or provider inquiries, requests, or concerns related to care plans.
* Communicates with care managers, practitioners, and others as needed to facilitate member services and ensure continuity of care.
* May support performing service assessments/screenings for members and documenting their care needs.
* Supports documenting and maintaining member records in accordance with state and regulatory requirements, and distributing information to providers as needed.
* Follows standards of practice and policies compliant with contractual, regulatory, and standard guidelines.
* Ability to identify needs and make referrals to Care Managers, community organizations, and Disease Managers.
* Provides education on benefits and resources available.

The pay range for this position is $25.00/hr.
