Position Purpose: Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, and continuity of care/member satisfaction is achieved. Interacts with members through telephonic outreach or home visits and documents care/service plans.
This is a remote position, but you must be in the office during training. After completing training and meeting performance metrics, you may work remotely. Residency in Florida is required.
Education/Experience: High School diploma or GED required; 1-2 years of related experience preferred.
License/Certification: For Florida - All interactions with members are telephonic.
Pay Range: $17.50 - $27.50 per hour
