Job Description: Care Coordinator

Position Purpose: Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, and continuity of care/member satisfaction is achieved. Interacts with members through telephonic outreach or home visits and documents care/service plans.

This is a remote position, but you must be in the office during training. After completing training and meeting performance metrics, you may work remotely. Residency in Florida is required.

  • Provides outreach to members via phone or home visits to discuss care plans, next steps, resources, questions, or concerns, and ongoing education throughout care/service.
  • Coordinates care activities based on care plans and collaborates with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress.
  • Supports inquiries, requests, or concerns related to care plans from members and providers.
  • Communicates with care managers, practitioners, and others to facilitate member services and ensure care continuity.
  • May assist in performing service assessments and documenting members' care needs.
  • Maintains member records per state and regulatory requirements and shares information with providers as needed.
  • Adheres to standards of practice, policies, contractual, and regulatory guidelines.
  • Identifies needs and makes referrals to Care Managers, community organizations, or Disease Managers.
  • Provides education on benefits and available resources.
  • Performs other duties as assigned and complies with all policies and standards.

Education/Experience: High School diploma or GED required; 1-2 years of related experience preferred.

License/Certification: For Florida - All interactions with members are telephonic.

Pay Range: $17.50 - $27.50 per hour
