Job Description
Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 30 locations to serve some of our community’s most vulnerable—and most resilient.

We are looking to hire a Care Coordinator to provide care coordination services to individuals enrolled in our One Care program—a program for patients with Medicaid and Medicare coverage between the ages of 21-64 years. You will work on a small, collaborative team with One Care Nurse Care Managers and other Care Coordinators as well as our medical teams to ensure that patients are being linked to needed services to improve their health and overall well-being and meeting goals outlined on their care plan.
In this role, you will also be responsible for spending approximately 20% of your time supporting administrative functions for the One Care team. A significant aspect of this work; however, will be direct patient engagement that will be a combination of telephonic and face-to-face outreach.
