Job summary

Compass Health is seeking a Care Coordinator in Everett, WA to provide wraparound, strength-based services to youth and families under WISe. This role partners with caregivers to promote recovery, coordinating care in natural settings such as home, school, and community.

The position requires a BA/BS/BSW in Behavioral Sciences, and experience in behavioral health or related fields is preferred. A valid WA driver’s license and ability to work evenings are required.

Qualifications

• BA/BS/BSW in Behavioral Sciences required.
• Experience in behavioral health, social services, or medical settings preferred.
• Familiarity with Evidence-Based Practices.
• WA State Driver's License and insured vehicle.
• Must complete Agency Affiliated Counselor application if not licensed.

Responsibilities

• Provide case management, treatment planning, and therapeutic coordination.
• Partner with caregivers and natural supports to promote recovery and stability.
• Deliver services in the community with flexible scheduling, including evenings.
• Participate in after-hours on-call rotation for crisis response and outreach.
• Serve as a core member of a multidisciplinary Red Team.

Are you passionate about empowering youth and families in your community? Our Everett WISe team is seeking a dedicated Care Coordinator to provide strength-based, wraparound care to children and young adults experiencing behavioral health challenges.

About WISe (Wraparound with Intensive Services)

WISe is a family- and youth-driven approach that supports Medicaid-eligible individuals up to age 21 with complex behavioral health needs. Services are provided in natural settings - home, school, and community - to help families thrive and avoid restrictive out-of-home placements.

What You'll Be Doing

• Provide case management, treatment planning, and therapeutic coordination.

What You'll Bring
