Description

Do you want to make a positive difference in the lives of youth and families? If yes, join the great team of people at Koinonia Family Services in Los Angeles (Gardena) working in our enhanced care management (ECM) program as a part-time ECM Care Coordinator. As a Care Coordinator, your role involves connecting individuals to services and resources best aligned to meet their unique needs in the greater Los Angeles area. You'll undertake assessments, develop individualized care plans, oversee service coordination and referrals, and provide education to equip individuals with the tools to make informed, healthy lifestyle decisions. Bilingual skills in English and Spanish are strongly preferred.

Enhanced Care Management is a statewide Medi-Cal benefit available to individuals with complex needs. The Care Coordinator (aka Lead Care Manager) will lead the assessment, care plan development and ongoing care management of eligible participants under the supportive guidance of an ECM Supervisor. Our program specifically targets foster youth and other at-risk youth populations, but may also include their family members or other adults. The Care Coordinator utilizes motivational interviewing to engage individuals to support them in better managing their chronic conditions. The Care Coordinator actively engages individuals to remove barriers to health, social services, and behavioral health care.

The Care Coordinator opportunity includes:

  • A job with a purpose; help support people in need
  • Hourly pay rate: Starting wage of $22.00 per hour, or higher with prior ECM experience
  • Benefits include paid time off (PTO), paid holidays, and participation in our 401k plan
  • Remote work available between in-person sessions
  • Work with a great team of people who care about each other and care about our mission to help youth and families find hope and healing
  • Mileage reimbursement for miles driven for business

This is a part-time role, initially offering 20–25 hours per week. Because this is a caseload-driven position, scheduled hours may increase or decrease over time based on client assignments and business needs. Applicants should be available to work at least 20 hours per week, although weekly schedules may vary (no minimum hour guarantee). As caseloads grow, additional hours may become available; if caseloads decrease, scheduled hours may be adjusted accordingly. Shifts are generally scheduled during normal business hours and may extend into the early evening based on member needs. Some scheduling flexibility is available to accommodate your personal schedule, provided client and business needs continue to be met.

Key Responsibilities:

Under the supportive guidance of the ECM Supervisor, you will work with the ECM team to provide services to youth and families as follows:

  • Conduct assessments of individuals' needs and formulate individualized care plans
  • Enthusiastically educate and enroll clients about services, and motivate them to engage in services.

  • Act as the primary liaison between individuals and their healthcare providers, and social services
  • Stay informed about community resources and actively refer individuals to appropriate services as needed
  • Maintain regular contact with clients to review and update care plans, and monitor clients' progress and outcomes
  • Advocate for the needs and rights of clients within healthcare and social service settings
  • Provide clients and families with resources and guidance on making informed health and lifestyle choices
  • Document services according to ECM program standards
  • Work closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery.

Qualifications:

  • A strong desire to help people in need achieve successful outcomes
  • Ability to speak Spanish
  • Minimum 2 years of case management (or related) experience

  • Demonstrated ability to work independently and maintain productivity with minimal supervision
  • Cultural sensitivity and awareness

As a licensed facility under Community Care Licensing, we are required to have applicants complete the California Live Scan process for a criminal history review. This process includes checks by the California Department of Justice, the FBI, and the Child Abuse Central Index. For positions in Los Angeles County, we consider qualified applicants with arrest or conviction records for employment in accordance with the Los Angeles County Fair Chance Ordinance for Employers and the California Fair Chance Act. In addition to criminal history, we review various aspects of an applicant's background, including but not limited to education, employment history, motor vehicle or driving history, reference checks, license or credential verification, and medical examinations.
