Description

As a Care Coordinator in the Emergency Department (ED) at Riverside Medical Center, you will play a critical role in ensuring efficient and effective patient care management from the point of entry through discharge or admission. Your primary responsibility is to coordinate and facilitate the delivery of services that align with the patient’s needs, hospital protocols, and insurance requirements, all while ensuring high-quality, compassionate care. Demonstrates flexibility with assignments within professional scope/duties/licensure.

Essential Duties

  • Patient Assessment & Coordination:
  • Conduct comprehensive assessments of patients' medical, social, and psychological needs.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Coordinate with physicians, nurses, and other healthcare professionals to ensure seamless patient care.
  • Resource Management:
  • Identify and address barriers to care, including financial, social, and logistical issues.
  • Facilitate the timely and appropriate use of resources, including consultations, tests, and follow-up care.
  • Ensure that patients and families are informed about their care plans and available resources.
  • Discharge Planning:
  • Develop and execute discharge plans considering patients’ home environments, follow-up needs, and community resources.
  • Arrange for post-discharge services, including home health care, outpatient therapy, and support services as needed.
  • Provide education to patients and families about discharge instructions and next steps.
  • Compliance & Documentation:
  • Ensure that all care management activities comply with hospital policies, regulatory requirements, and payer guidelines.
  • Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress notes.
  • Assist with the documentation needed for insurance authorizations and billing.
  • Collaboration & Communication:
  • Act as a liaison between patients, families, healthcare providers, and external agencies.
  • Participate in multidisciplinary team meetings to discuss patient progress and care plans.
  • Advocate for patients' needs and preferences within the care team.

Requirements

Preferred Experience:

  • Three years of clinical nursing with diverse patient experiences regarding chronic conditions, age, and cultural background, or 1year of nursing experience and 5 years in related health field with an opportunity to develop relevant care management and coordination skills.
  • Relevant clinical experience and current knowledge of healthcare trends related to models of population health management; knowledge of chronic conditions and evidence-based medicine; basic understanding of principles of care management and care coordination as well as psychosocial and behavioral factors affecting health.
  • Basic proficiency in Microsoft Excel and Word and ability to navigate and troubleshoot basic computer operations.

Required Licensure/Education

  • Current Illinois State License as a Registered Nurse
  • Basic Life Support (BLS) certification required.
  • *New Graduate RNs starting on or after 1/1/25 must earn Bachelor’s of Science in Nursing (BSN) within 3 years of start date.

Preferred Education

  • Bachelor's Degree in Nursing preferred*

Lifting equipment - carry 25'- 150' waist high (IV bags, patient trays). Carry charts 25'-150' waist high. Filled laundry bags are filled half full so as not to weigh over 35 lbs. Carry fire extinguishers up or down 3-4 flights of stairs and several yards in an emergency.
