Job description

Overview

Emergency Department (ED) Nurse Care Manager is responsible for establishing, coordinating, and maintaining processes to optimize patient throughput to the most appropriate level of care. They facilitate interdisciplinary care across the continuum for the ED by collaborating with patients, families, multidisciplinary teams, physicians, community resources, and payers to ensure appropriate care levels and efficient, cost-effective treatment.

Responsibilities

Include, but are not limited to:

  1. Level of Care Determination: Conducts initial medical necessity reviews for ED patients with potential admissions, collaborates with providers to identify appropriate care levels, documents per policy, and educates ED staff on criteria.
  2. Refers High-Risk Patients to Social Services: Uses screening criteria to refer patients with complex psychosocial or medical discharge needs to social workers.
  3. Implements Actions to Prevent Readmission: Recommends outpatient or community care referrals, conducts follow-up calls, and develops interventions for high utilizers.
  4. Supports Patient Flow: Expedites diagnostics, offers alternatives to admission, and participates in boarder rounds to adjust dispositions.
  5. Initiates Discharge Planning: Conducts social determinants screening and initial discharge assessments for ED-admitted patients.
  6. Maintains Knowledge of Regulations: Keeps updated on federal, state, and accreditation standards, including CMS rules, observation status, and Medicare procedures.
  7. Provides Education and Resources: Educates patients and staff on care progression, utilization, and safe transitions.
  8. Identifies Risks and Quality Issues: Works with the healthcare team to resolve or escalate issues as needed.

Other duties as assigned by management.

Qualifications
  • BSN or related Bachelor’s Degree.
  • Clinical experience in inpatient care management.
  • Experience in acute care utilization preferred.
  • Utilization review or case management training preferred.
  • RN license required (NJ).
  • Case Management certification preferred.
  • Strong data and computer skills.
  • Knowledge of utilization review and appeals regulations.
