Job description

The Care Coordinator role at UnitedHealthcare is focused on simplifying the healthcare experience and improving community health by removing barriers to quality care. This position involves managing a panel of older adult members with complex medical and behavioral health needs. The Care Coordinator will support members' medical, behavioral, and socioeconomic needs to ensure appropriate service utilization and improved care quality.


Key Responsibilities:



  • Serve as the primary care manager for high-risk members with comorbid behavioral health needs.

  • Engage with members through face-to-face and telephonic interactions to complete comprehensive needs assessments.

  • Develop and implement individualized care plans that address medical, behavioral, functional, cultural, and socioeconomic needs.

  • Collaborate with internal care teams, providers, and community resources to execute care plans.

  • Provide referrals and linkages to appropriate services and resources.


Qualifications:



  • Master's or Bachelor's degree in social work, psychology, special education, or counseling, with relevant case management experience.

  • Current RN or LPN licensure in Indiana with clinical experience.

  • Experience with local behavioral health providers and community support organizations.

  • Proficiency in MS Office and experience in long-term support services.

  • Resident of Indiana with reliable transportation.


Preferred Qualifications:



  • Additional experience in long-term support services, team-based care, and working with co-occurring disorders.

  • Training in Motivational Interviewing, Trauma-Informed Care, and Person-Centered Care.

  • Background in Managed Care and bilingual abilities.


Physical Requirements:



  • Ability to transition between office and field locations.

  • Capability to navigate various terrains and remain stationary for extended periods.
