Cleveland Clinic Care Coordinators have beenvery successfulin helping patientsmanage their own care.In this role, yourhard work,dedicationandcommitmentcanlead to a decrease in Emergency Department visits, observation status, inpatientstaysand hospital readmission in care coordinated patients.
A caregiver in this positionworksa hybrid schedule from8:00am to 4:30pm or 8:30am to 5:00pm, with the availability to work from home two days per week.This role will be stationed at Crile Building at CC Main Campus.
A caregiver who excels in this role will:
Work collaboratively with a multidisciplinary care team across the continuum of care for high-risk patients to develop goals, planinterventionsand maximize patient outcomes.
Provide care and disease management coordination.
Identifypatients in the specialty care practice that have ongoing coordination needs and conduct targeted outreach.
Utilize assessment skills, risk assessmenttoolsand technological tools toidentifypatients with care needs and manage populations.
Conduct comprehensive clinical assessments that include disease-specific, age-specific, medical, behavioral, pharmacy, social andend of lifeneeds of each patient.
Inform and work with patients and their familiesregardingcoordination of their care, provide education and coaching,monitorpatient compliance with their care plan, perform reassessmentsregardingpatient progress toward goals, and update plan of care.
Serve as a liaison and advocate for patients and families.
Assistin managing transitions of care across care settings, ensuringoptimalcommunication and planning.
Identifybarriers,facilitatesolutions, and connect others to community resources.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accreditedschool ofProfessionalNursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) Certification through the American Heart Association (AHA) Or the American Red Cross
Three to five years of nursing experience
Preferred qualifications for the ideal future caregiver include:
Bachelor’sofScience inNursing (BSN)
