Empowering Minds Resource Center, LLC. in Baltimore, MD seeks an Internal Staff Full Time Client Service role to provide targeted mental health case management for youth with serious emotional disturbances and co-occurring disorders.

You will meet with clients and families to develop Plans of Care, perform assessments per DHMH requirements, and coordinate Family Team Meetings. You will maintain documentation in the EHR and be available for on-call responses and group facilitation.

Formación

• Bachelor's degree in a mental health field plus 1 year of mental health experience, including peer support.
• Bachelor's degree in a field other than mental health with 2 years of mental health experience, including peer support.
• Ability to handle stressful situations.
• Flexible work schedule to meet client needs.
• Care Coordinator training and CSA/BHA training requirements including CASII, ESCII, and CANS.

Responsabilidades

• Provide targeted mental health case management for youth with serious emotional disturbances and co-occurring disorders.
• Meet with enrolled and potential clients and their families to create a Plan of Care.
• Conduct comprehensive assessments and other assessments as required by DHMH.
• Coordinate and facilitate Family Team Meetings.
• Provide management of the POCs.
• Collect information during the application process and as identified in each POC.
• Identify providers, supports, and resources for clients.
• Coordinate meetings with client and family to ensure providers are a good fit for proposed services and products in compliance with the POC.
• Maintain client clinical documentation using EHR.
• Provide on-call services as scheduled.
• Promptly respond to client crises.
• Provide case management for client open-access as scheduled.
• Facilitate groups.

• Provide targeted mental health case management for youth with serious emotional disturbances and co-occurring disorders
• Meet with enrolled and potential clients and their families to create a Plan of Care

• Coordinate meetings with client and family to (a) meet with providers to ensure goodness of fit for proposed services and products in compliance with the POC (b) meet with family peer-to- peer support, intensive in-home service, mobile crisis response service, and other providers appropriate to the POC
• Maintain client clinical documentation using EHR
• Provide on-call services as scheduled
• Promptly respond to client crises
• Provide case management for client open-access as scheduled.
• Facilitate groups

Minimum Qualifications:

• Bachelor's degree in a mental health field and 1 year of mental health experience, including mental health peer support; or Bachelor's degree in a field other than mental health and 2 years of mental health experience, including mental health peer support
• Ability to handle stressful situations.
• Must maintain a flexible work schedule to meet scheduling needs of clients
• Must meet Care Coordinator training and certification requirements
• Must complete the training required by CSA and BHA, including CASII, ESCII, and CANS
