Job summary

Cumberland County Guidance Center seeks a Full-Time Care Coordinator to join Project Redirect, helping individuals access behavioral health services and community resources to engage in treatment and reintegration.

The role emphasizes liaison work between providers, criminal justice partners, and program participants, with responsibilities across outreach, coordination, and documentation to support project success.

Qualifications

Knowledge of behavioral health services and community resources
Strong communication, engagement, advocacy, and relationship-building skills
Ability to work independently and as part of a multidisciplinary team
Strong organizational and documentation skills
Possession of a valid New Jersey driver’s license with clean driving record
Ability to work a flexible 40-hour schedule

Responsibilities

Coordinate services between behavioral health providers, criminal justice agencies, and program participants
Assist individuals in accessing mental health treatment and community resources
Conduct assertive community outreach and engagement activities
Monitor treatment participation and support adherence to program requirements
Collaborate with clinicians, courts, and community partners to support outcomes
Promote access to housing, benefits, healthcare, employment and other resources
Assist with data collection, reporting, and program documentation
Attend court proceedings to identify and engage participants
