Qualifications

• Knowledge and experience in Hi-Fidelity Wraparound preferred.
• Experience working with families connected to behavioral health.
• Must have a valid Driver's License.

Responsibilities

• Identify care coordination needs/services with families.
• Engage with children, youth, and families in care planning.
• Develop partnerships with community resources to support families.
