Position Description

The Care Coordinator, under the supervision of the Program Director, Assistant Program Director, Clinical Care Manager, and/or Nurse Care Manager, will provide outreach, engagement, assessment, and person-centered treatment planning for individuals enrolled in the Brien Center’s Behavioral Health Community Partner (BHCP) Program. Responsibilities include care coordination and management services, engaging with Enrollees to facilitate care integration, support transitions, and deliver treatment for individuals with Serious Mental Illness, co-occurring disorders, and Substance Use Disorders, in collaboration with the Interdisciplinary Team and BHCP staff.

Qualifications

• Associates or Bachelor’s Degree, or 4 years of relevant human service or healthcare experience
• Experience in community-based behavioral health programs preferred
• Knowledge of the Stages of Change Model, Substance Use Disorder treatments, and recovery models
• Willingness to view substance use treatment as chronic disease management
• Ability to pass Criminal Offender Record Information and RMV checks
• Valid driver’s license, reliable transportation, and insurance required
