Care Coordinator builds an alliance with clients by providing education, and motivation to support them in making positive changes in their lives and reducing the negative consequences and barriers associated with substance use. The Care Coordinator provides client-centered care utilizing evidence-based and trauma-informed services. They use motivational interviewing techniques to establish therapeutic rapport and facilitate active engagement in services. They conduct assessment interviews using the ASAM and DSM-5. They work collaboratively and provide evidence-based therapeutic interventions to address the health service-related needs (HRSNs) and barriers of the client. They facilitate evidence-based groups and conduct individual counseling sessions, monitoring progress towards identified goals of the client.
Associate degree or higher in Human Services or a related field from an accredited college or university.
In addition to formal education, the qualified candidate will generally have at least one to three years of drug and alcohol addiction counseling experience.
CADC I certification or higher with MHACBO.
Qualified candidate must be able to pass a DHS criminal background check.
Excellent interpersonal communication skills. Strong computer and time management skills. Advanced understanding of ASAM PPC-2R and DSM-5. An intermediate understanding of Oregon Administrative Rules is preferred. Qualified candidate must be able to pass a DHS criminal background check.

Wage will depend on experience and credentials. An additional stipend is available for Spanish/English bilingual employees following the completion and passing of the Alta Language Test.
