Oscar Health is seeking a Care Coordinator to join our Care Management team. You will serve as the central entry point for members and providers, coordinating care across CCM, RAP, and unassigned populations while supporting RN Care Managers.

This remote role serves residents of AZ, FL, GA, and TX, with occasional travel for team meetings and events. You will work with Medecision EMR and InContact to streamline workflows and enable one-call resolution.

Qualifications

• 1+ years of experience with care coordination and navigation in a healthcare setting.
• Proficient with EMRs like Medecision and telephony systems like InContact.
• Excellent interpersonal communication and listening skills.

Responsibilities

• Be the primary bridge and entry point for all members and providers working with the Care Management team.
• Field inbound phone calls from members and providers, using optimized IVR pathways to ensure accurate routing and striving for one-call resolution.
• Manage care coordination for members across programs (CCM, TOC, unassigned populations) across various settings while maintaining confidentiality and professionalism.
• Manage non-emergent medical transportation (NEMT) via UberHealth following program guardrails to reduce readmissions.
• Handle internal escalations to Claims, Eligibility, and Care Navigation.
• Lead special projects as subject matter expert.
• Collaborate cross-departmentally to enhance member experience.

About the role:

Care Coordinators serve as the central entry point and operational backbone for the Care Management department. They provide comprehensive care coordination for members, including those in Complex Care Management (CCM), Readmission Prevention (RAP), and those who do not fit into existing highly specialized programming. By taking ownership of administrative tasks, managing post-discharge transportation needs, and resolving inbound inquiries with a focus on one-call resolution, the Care Coordination Team allows our RN Care Managers to operate at the top of their license and helps members reach their optimum health goals in the most cost effective way.

Work Location: This is a remote position, open to candidates who reside in: Arizona; Florida; Georgia and Texas. While your daily work will be completed from your home office, occasional travel may be required for team meetings and company events.

Pay Transparency: The base pay for this role is: $17.91 - $23.51 per year. You are also eligible for employee benefits, monthly vacation accrual at a rate of 15 days per year.

• Manage non-emergent medical transportation (NEMT) via UberHealth for members, strictly following program guardrails to decrease readmissions and improve health outcomes.
• Handle internal escalations to specialized departments such as Claims, Eligibility, and Care Navigation.
• Lead special projects as subject matter experts.
• Collaborate cross-departmentally to create a positive member experience.
• Perform all other tasks or additional responsibilities.

Requirements:

• 1+ years of experience with care coordination and navigation in a healthcare setting.
• Proficient computer literacy skills, with a strong aptitude for using EMRs (like Medecision) and telephony systems (like InContact) for seamless workflows.
• Excellent interpersonal communication skills including exceptional listening skills.
