Job summary

Seasons Center for Behavioral Health in Sheldon, IA is seeking a full-time Care Coordinator to coordinate service provision for clients, facilitate communication between providers, and ensure individuals receive appropriate care for their needs.

The role requires a bachelor’s degree in social work or related field, with preferred Iowa BSW license; experience is a plus. Monday–Friday 8am–5pm, strong benefits and PTO included.

• Bachelor’s degree in social work, human services, or a related field is required.
• Experience coordinating care for individuals with behavioral health needs is a plus.

Responsibilities

• Coordinate service provision for clients across providers.

Pay Range: $22.00-$26.00 per hour

Schedule: Monday-Friday 8:00am-5:00pm

About the Job

Seasons is a comprehensive behavioral health center offering a broad range of psychiatric and behavioral health services to communities in the Northwest Iowa region. We’re currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers, facilitating communication between providers, and ensuring individuals receive the appropriate care for their individual needs.

Requirements

• A Bachelor’s degree in social work, human services, or a related field is required
• Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required
• An active Iowa Bachelor’s of Social Work (BSW) license is preferred but not required
