Job summary

Pomelo Care is a leading virtual medical practice delivering 24/7 clinical care for women and children nationwide. We are seeking an operations-focused teammate to own essential administrative processes that drive clinical efficiency and patient satisfaction.

You will collaborate with clinicians to facilitate care navigation, eligibility checks, scheduling, and care coordination, while supporting a flexible remote schedule within the U.S.

Qualifications

• 2+ years healthcare administration experience.
• Familiarity with SDOH platforms and referrals.
• Ability to work cross-functionally with clinicians.

Responsibilities

• Provide care navigation support for patients finding in-network providers or referrals to social services.
• Monitor member support line and answer patient questions.
• Perform eligibility checks and manage patient offboarding.
• Own patient scheduling, reminders, and provider schedule optimization.
• Coordinate referrals to health plan Case Management.
• Develop workflows with Market Operations to improve care coordinator efficiency.
• Handle other duties as assigned.
• Maintain availability Monday–Friday, 9:00 AM–6:00 PM ET.

About The Role:

Your north star: Support the Pomelo clinical practice's efficiency by owning key operational & administrative processes.

What You'll Do

• Work closely with clinical staff to provide care navigation support for patients who need help finding providers in their network and/or being connected to other care or social services such as housing, transportation, WIC and SNAP.
• Monitor our member support phone line and answer any questions that patients call in with
• Complete eligibility checks for new and existing patients and own the patient offboarding process
• Own patient scheduling, including patient prioritization, appointment reminders and provider schedule optimization
• Manage referrals to health plan's Case Management team
• Collaborate with Market Operations Lead to develop new workflows focused on improving care coordinator efficiency
• Other responsibilities as assigned
• Be available to work a set schedule of Monday to Friday from 9:00 AM to 6:00 PM ET

• 2+ years healthcare administration experience, ideally having worked within virtual care
• Familiarity with SDOH platforms and referrals

Compensation

The expected salary range offered for this role is $21.63 to $24.04 an hour ($45,000 to $50,000 if annualized). This role is also eligible for equity, giving you an ownership stake in Pomelo's mission. Actual compensation may vary based on relevant experience, skills, competencies, and certifications.
