Job summary

- Bilingual skills preferred (Creole and/or Spanish).

Education
Master's Degree in Social Work or related field
Position Definition:Provides client management services, from assessment of needs to coordination for referrals of all identified needs which may include: behavioral health services, youth development programming, mentorship, recreational programing, and others as identified.
Example of Duties:Interviews young people referred by various agencies to assess behavioral health needs and social service needs. Determines the appropriate type of remedial service and prepares a plan for the client detailing the programs and services to be used. Reviews recommendations with supervisor prior to implementation. Refers the client to appropriate programs and services. Follows-up with the client to insure appropriateness of identified needs and to substantiate connections to referred programs and services. Monitors client’s on-going participation in the recommended programs and services to completion. Evaluates the effectiveness of the recommended programs and services. Documents case information to ensure services can be identified, tracked, and evaluated for appropriateness, efficacy, and issue trending.
Additional Duties: Prepares and maintains confidential case records and related files. Maintains working relationships with the Department’s Restorative Justice Staff, as well as community providers and various other agencies such as The Department of Children & Families, Norwalk Parks & Recreation Dept., and Norwalk Dept. of Health. Attends departmental meetings as assigned. Operates general office and computer equipment. Performs other related duties as assigned.
Supervised By:This position reports to and receives direction from the Director of Youth Services.

Ability to ensure youth and families are connected with the identified services deemed appropriate upon completion of assessment and in coordination with Departmental review. Ability to support some direct service programming as may be identified and/or developed internally. Ability to maintain and develop viable working relationships with other youth serving agencies. Ability to maintain confidential records.
