Care Coordinator

Bedford‑Stuyvesant Restoration Corporation, founded in 1967, is the nation’s first community development corporation located in the heart of Central Brooklyn. The organization operates Restoration Plaza, a 300,000‑square‑foot commercial complex that offers retail, dining, recreation, and entertainment. The mission is to close the racial wealth gap by providing direct services across an economic mobility platform centered on workforce development, affordable housing/homeownership, community revitalization, small business development, and arts and culture to 60,000 people annually.

About the Role

The Care Coordinator will provide holistic and comprehensive case management services to all participants, ensuring access to wraparound supports that promote financial stability, employment, and overall well‑being.

Key Responsibilities
  • Conduct intake, triage, and assessments across domains including benefits, financial health, education, employment, and social environment.
  • Complete Financial Health Assessments and PHQ‑9 screenings for depression, anxiety, stress, and substance use during the initial evaluation process.
  • Develop individualized service and goal plans to connect clients with benefits enrollment, financial counseling, education and training, and job placement or retention support.
  • Identify barriers to asset building and financial independence, such as unemployment, lack of childcare, limited education, or housing instability, and make referrals to internal and external resources.
  • Conduct intake and triage sessions with 100% of newly assigned participants and determine subsequent steps to help them achieve short‑ and long‑term goals.
  • Ensure 100% of unemployed or underemployed clients are referred and scheduled for a Career Development Specialist.
  • Ensure 100% of participants are scheduled for Financial Counseling sessions.
  • Document all participant interactions, progress updates, and outcomes in accordance with organizational and funder requirements.
  • Follow up on declined referrals using motivational interviewing to support behavior change and engagement.
  • Inform clients of community events, workshops, and programs such as Restoration Outreach events.
  • Follow up on all referrals and document outcomes, maintaining thorough case notes reflecting participant progress.
  • Fulfill all contractual obligations and program performance benchmarks.
  • Utilize Restoration’s Database, Salesforce, and Google Suite applications for accurate data entry and case tracking.
  • Perform other duties as assigned.
What You Bring to the Role
  • Bachelor’s Degree with 2–3 years of social service experience preferred.
  • Experience with benefits eligibility screening (e.g., SNAP, health insurance, SCRIE, DRIE) is a plus.
  • Knowledge of community resources and counseling or social work practices serving underserved populations.
  • Strong written and verbal communication and documentation skills.
  • Ability to establish rapport and motivate clients toward goal achievement.
  • Demonstrated ability to work independently and manage multiple priorities in a fast‑paced environment.
  • Strong confidentiality, integrity, and professional boundaries.
  • Works occasional evenings and weekend hours as needed.
  • Bilingual (Spanish/English) preferred but not required.
  • Cultural competence and ability to work effectively with diverse families and communities.
  • Proficiency in Google Workspace and basic office software.
Hourly Rate

$24.73 – $27.47
