Overview

Our client is seeking a highly motivated and empathetic individual to join our team as a Care Coordinator. In this critical role, you will be the first point of contact for patients, physicians, and referral sources, providing compassionate support and efficient, first-call resolution.

Location: Remote
Hours: Flexible between 7:00 a.m. and 7:00 p.m. CST
Rate: $20.00 per hour

Responsibilities
  • Process Patient Applications: Create and complete accurate referrals and applications using our centralized Customer Relationship Management (CRM) platform, staying updated on policy or procedural changes.
  • Resolve Patient Concerns: Answer patients' questions and concerns regarding their patient journey status with clarity and empathy.
  • Maintain Detailed Records: Enter detailed, accurate, and clear information into our company proprietary software while conversing with patients via telephone.
  • Conduct Outbound Communication: Place outbound phone calls for patient follow-ups or confirmations to ensure timely progression of their accounts.
  • Provide Empathetic Support: Maintain high-quality service while providing an empathetic and supportive experience, guiding the caller effectively.
  • Manage Referral Interaction: Interact with patient referral sources, including healthcare providers, to efficiently process new applicants.
  • Steward Patient Accounts: Take ownership of patient accounts from initial contact through final approval or denial, ensuring a seamless experience.
