Thisposition is responsible for performing duties related to patient care coordination. This position is designated as an on-site, patient-facing role. In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA.
Work schedule is Monday-Friday, 8:00 am to 5:00 pm with a one-hour lunch. Some after-hours work may be necessary to achieve the goals of the organization. Travel to other sites of Eastern Nephrology Associates. Slight modifications may be made to this schedule with approval from the Value Based Care Manager. All schedule changes must be requested in writing 30 days in advance to the Value Based Care Manager and Human Resources.
