Job summary

Hazelden Betty Ford Foundation in Minnesota is seeking a Care Coordinator to coordinate Recovery Management across the continuum of care, directly working with patients to develop continuing care plans and ensure smooth transitions through our services for families and referents.

As part of the treatment team, you will create a patient-focused experience, support continuum of care development, and help Hazelden Betty Ford Foundation deliver evidence-based care in a mission-driven environment

Qualifications

• High school diploma or equivalent required.
• 2+ years in customer service.
• MS Office proficiency including Outlook, Word, PowerPoint.
• Experience in online research.
• Current CPR certification from American Red Cross or American Heart Association.

Required Qualifications

• High school diploma or equivalent
• 2 or more years in customer service
• Intermediate level experience with Word software, including Outlook, Word, PowerPoint
• Experience in online research
• Current CPR certification.
  • Certification must be from American Red Cross or American Heart Association.

Preferred Qualifications

• Vocational or associates degree
• Experience with patient database
• Basic knowledge of chemical dependency, including Twelve Step abstinence-based treatment programs.
• Experience in a behavioral care setting
