Primary duties include regular engagement with participants, developing care plans, and coordinating with local agencies to support housing, employment, and recovery goals, while documenting interactions for quality and compliance.

Qualifications

• Bachelor's degree in a related field OR four years' direct human services experience.
• Possession of a valid Driver’s License.
• Ability to successfully complete a background check.
• Commitment to complete the State of North Dakota care coordination training after hire.
• Preferred: Case management experience.
• Preferred: Excellent writing skills.
• Preferred: Self-starter.
• Preferred: Experience in case management, parole/probation, or addiction services.

Responsibilities

• Direct Engagement: Conduct regular, weekly meetings with program participants to monitor progress.
• Care Planning: Develop participant-centered care plans.

Position Summary

The Care Coordinator plays a vital role in supporting individuals involved in our specialized programs. The primary goal is to empower clients to reach and maintain their highest level of physical and mental well-being by providing participant-centered care and coordination with local agencies. You will act as a primary advocate, helping individuals navigate the realms of recovery, housing, and employment.

Key Responsibilities

• Direct Engagement: Conduct regular, weekly meetings with program participants to monitor progress and provide guidance.


• Care Planning: Assess individual needs and develop a participant-centered care plan focused on client-driven goals.


• Inter-Agency Collaboration: Partner with local agencies and care teams to ensure success in critical areas, including recovery, housing, employment, and reducing recidivism.


• Documentation: Maintain timely and accurate documentation of each client interaction to ensure high-quality care and compliance.

Required Qualifications

• Education/Experience: A Bachelor's degree in a field closely related to the position's responsibilities OR four (4) years of experience providing direct services in a human service setting.


• Licensure : Possession of a valid Driver’s License.


• Vetting: Ability to successfully complete a background check.


• Training: Commitment to complete the care coordination training provided by the State of North Dakota after being hired.

Compensation And Benefits

• Job Type: Full-time / Part-time


• Pay: Starting at $4,000 per month. Paid Monthly


• Compensation is based on outcomes for each participant.


• Work Schedule: Flexible work hours. Care Coordinators schedule their own appointments, offering significant professional freedom and flexibility.
