Qualifications

• Master's degree in Counseling, Social Work or Psychology is required.
• Licensed Clinical Social Worker (LCSW) license is required; LMFT and LPC are preferred.
• Minimum 3 years in case management; 5 years in acute care; 1 year with vulnerable populations; supervisory experience preferred.

Responsibilities

• Lead daily operations and coordinate work within the Department of Managed Care at the assigned site.
• Provide educational services and clinical oversight to the Care Coordination team.
• Participate in team activities and deliver care coordination services as needed.
• Mentor staff, coach performance, and ensure licensing and quality standards are met.
