This role focuses on patient flow, coordination of services, and ensuring timely communication across teams. You will help coordinate medical transportation, monitor tracking, and assist with Medicare communications, contributing to a patient-centered experience and efficient care delivery.

Qualifications

• Associates degree required; Baccalaureate Degree preferred.
• Computer skills including Microsoft Office required.
• EPIC, Midas, and NaviHealth experience preferred.

Responsabilités

• Provides support of daily activities of the Care Coordination Department.
• Coordinate medical transportation and tracking, along with delivery of the 2nd Important Message of Medicare.
